Annual Summary Reports

A 425 PEIMS record is required for each disciplinary action taken against a student which results in the removal from the regular classroom for at least one day.  A single student can have multiple records if removed from the classroom more than once and a single incident can result in multiple actions.   These data are required by TEC Chapter 37, and by IDEA 2004, which includes students who are receiving special education and related services.

Every disciplinary action that results in the removal of a student from any part of their regular academic program is categorized in one of the following general categories:  In-School Suspension (ISS), Out-of-School Suspension (OSS), Expulsion, Juvenile Justice Alternative Education Program (JJAEP), or Disciplinary Alternative Education Program (DAEP) assignments. 

The counts on these reports are generated independently at the state, region and district levels, depending on the level requested.  In general, if the heading name includes “students”, a student is counted once for the particular criteria of action or reason at the selected level.  Otherwise, this is a count of records and students may be counted more than once.

In Section W – Reason Incident Counts, a disciplinary incident number is generated when an incident occurs on a campus which results in one or more disciplinary actions for one or more students.  In some cases, a student may receive more than one disciplinary action for the same disciplinary offense resulting in a district submitting more than one record for the identical incident number.    

A disciplinary incident number is counted once at the campus level.  The counts at the state, region and district levels are created from the summation of the campus counts.

In Section X – Discipline Action Counts, for each discipline action code a total count of records is created for the selected level of state, region, or district.  A student may have multiple records.
