Annual Student and
Incident Counts
A 425 PEIMS record is required for each disciplinary action
taken against a student which results in the removal from the regular classroom
for at least one day. A single
student can have multiple records if removed from the classroom more than once
and a single incident can result in multiple actions. These data
are required by TEC Chapter 37, and by IDEA ’97, which includes students who are
receiving special education and related services.
A disciplinary incident number is generated when an incident
occurs on a campus which results in one or more disciplinary actions for one or
more students. In some cases, the
circumstances surrounding a particular student’s disciplinary offence may result
in a district submitting more than one record for the same student with
identical incident numbers and reason codes but different action codes. The disciplinary incident number is only
unique at the campus level.
STUDENT COUNTS
Students are counted once at the state, region or district
level for a disciplinary action and disciplinary action reason. Student counts less than 5 but greater than zero
are masked at all levels. In certain cases, a student count
greater than or equal to five may be masked in order to ensure the
privacy of an individual.

Incidents are counted once for a disciplinary action and disciplinary
action reason. Incident counts less than 5 but greater than zero are
